Important Documents to Bring for your Tax Preparation
Please do not use staples, tape or highlighters.
If itemizing: List all cash/non cash donations, medical expenses, prescriptions and business expenses NEATLY on a separate sheet of paper. Keep all receipts for your records.
- Current Photo ID for both Yourself and/or Spouse. (New clients only)
- Copy of Previous Year State & Federal Tax Return . (If not prepared by our office)
If necessary, contact the IRS for a copy of last year's return if you don't have it.
- W-2 forms from each employer.
- All 1099 forms (1099-R, 1099-INT, 1099-DIV, 1099-B, 1099-G, 1099-MISC) showing interest, dividends, security sales, etc.
- Energy Credits - {MUST BRING} All Purchase Documents.
- Documents showing the original purchase price of assets sold during the year, such as stocks, bonds or home. Brokerage Statements or 1099-B.
- All forms indicating federal and state tax paid, including Estimated Taxes.
- If you received Social Security Benefits, bring your Annual Statement, SSA-1099.
- If you received a pension or annuity, bring your 1099R.
- If applicable, unemployment compensation statements, 1099-G.
- Self employed: List all expenses on a separate sheet of paper, total each category.
- Schedule K-1 Dividends, Interest, Capital Gains & Royalties.
- Child Care provider information (name, address, employer ID or SSN and amount paid for each child.)
- Voided Check if you want your refund electronically deposited. [NOT A DEPOSIT SLIP.]
- Proof of Health Care Coverage: Form 1095-A if you purchased healthcare through the Health Insurance Marketplace.
Please remember that we are assisting you with your Tax Return. It is your responsibility to make sure we have completed your return accurately and all information has been included.
